Sales Invoice

NOTICE: Seller has no responsibility for collection of Sales or Use taxes imposed by the state of IN.

Lock Joint Tube LLC hereby certifies that these goods were produced in compliance with all applicable requirements of section 6, 7 and 12 of the Fair Labor Standards Act, as amended, and all Regulations and orders of the United States Department of Labor issued under Section 14 thereof.

Sales Agreement (Confirmation)

No claims allowed unless made within thirty days of receipt of goods.

This is to certify the merchandise listed in this invoice has been produced under the Fair Labor Act of 1938 as amended.

Per ISO 9001:2015 registration, controlling documents for contract acceptance will be our Advance Quality Planning and ECR. These customer signed documents will preclude and take precedence over all other correspondence relating to contract requirements.

Orders are considered complete at plus or minus ten percent of the total amount ordered.

Material Certificate Report

Mechanical tubing supplied is certified to be in compliance with the latest revision of ASTM A513. Type 1 for hot-rolled products and type 2 for cold-rolled products. Metallic coated mechanical tubing supplied is certified to be in compliance with the latest revision of ASTM A787. Structural tubing supplied is certified to be in compliance with the latest version of ASTM A500. Metallic coated structural tubing supplied is certified to be in compliance with the latest version of ASTM 1076. The above holds true for all products unless otherwise agreed upon between Lock Joint Tube LLC and the customer.

The chemical analysis of raw material are furnished by the supplier and are certified to be correct as contained in the records of said corporation. Where required, mechanical property values are derived from actual test results.

Bill of Lading

The following provisions, and the provisions set forth at the foot of this document, do not apply if this is a contractor private carriage. The property described below, in apparent good order, except as noted (contents and condition of contents of packages unknown), marked consigned, and destined as indicated below, which said carrier (the word carrier being understood throughout this contract as meaning any person or corporation in possession of the property under the contract) agrees to carry to its usual place of delivery at said destination if on its route, otherwise to deliver to another carrier on the route to said destination.

It is mutually agreed as to each party at any time interested in all or any of said property, that every service to be performed hereunder shall be subject to all the terms and conditions of the Uniform Domestic Straight Bill of Lading set forth (1) in Uniform Freight Classification in effect on the date hereof, if this is a rail or rail-water shipment, or (2) in the applicable motor carrier classification or tariff if this is a motor carrier shipment. Shipper hereby certifies that he is familiar with all the terms and conditions of the said bill of lading including those on the back thereof, set forth in the classification or tariff which governs the transportation of this shipment, and the said terms and conditions are hereby agreed to by the shipper and accepted for himself and his assigns.

Subject to Section 7 of Conditions of applicable bill of lading. If this shipment is to be delivered to the consignee without recourse on the consignor, the consignor shall sign the following statement: The carrier shall not make delivery of this shipment without payment of freight and other lawful charges.

This is to certify that the above named materials are properly classified, described, packaged, marked and labeled, and are in proper condition for transportation.

NOTE – Where the rate is dependent on value, shippers are required to state specifically in writing the agreed or declared value of the property. The agreed on declared value of the property is hereby specifically stated by the shipper to be not exceeding [blank].

The fibre boxes used for this shipment conform to the specifications set forth in the box maker’s certificate thereon, and all other requirements of the Uniform Freight Classification.

Received, subject to the classifications and lawfully filed tariffs in effect on the date of the issue of the Bill of Lading.

The terms and conditions of the Uniform Straight Bill of Lading are hereby incorporated by reference.

Purchase Order

If the material on this order is not shipped on or before the time specified herein, the purchaser has the privilege of canceling this contract without notice.

Please advise receipt and acceptance of this order promptly.

Goods subject to our inspection on arrival, notwithstanding prior payment to obtain cash discount.

Goods rejected on account of inferior quality or workmanship will be returned to you with charge for transportation both ways plus labor, reloading, trucking,

etc., and are not to be replaced except upon receipt of written instructions from us
